CHECK LIST FOR CONSUMERS  Don't forget flood insurance when planning your financial future.

In order to get flood insurance coverage that is right for you, complete the following CHECK LIST, print it out and have it available when you call your insurance company or agent to get a quote on flood insurance.

  1. What is the name of the community where the building you want to insure is located?

  2. Have there been any major improvements or additions made?

  2. If you own the building, when was it purchased?
  3. If you own the building, do you have a mortgage?

If yes, name and address of mortgage company:

  1. What type of building is it?
    Single Family Condominium Business?
  2. Are there additional structures or buildings that you want to insure?
    Yes No

If so, explain:

  1. Does the building have a basement? (All sides sub grade or below ground level)

  2. Is the property located near a lake, river or ocean?
    Yes No

  1. Do you know of any history of flooding in you community?
  2. Has your home ever been flooded?
